Our dental membership plan has no waiting periods, no deductibles, no yearly maximum, and no pre-existing conditions.
The Smile Care Plan is not dental insurance and is not intended to replace dental insurance or health insurance. It is only valid at our office.
How Much Does the Smile Care Plan Cost?
Patients can join the Smile Care Plan at just $495/year! Membership plan enrollment will then cost $395/year for the second adult patient in the family, and $295 per child (age 20 years old and under).
What Does the Plan Include?
Smile Care Plan members at Derek H. Tang, DDS enjoy the following benefits and savings:
- Two preventive check-up exams per year
- Two routine or periodontal cleanings per year
- All x-rays associated with check-up exams
- Free dental consultations
- Two additional dental cleanings at 10% off our current fee schedule
- 10% off our current fee schedule for additional dental services
- Two emergency exams per year
Terms & Conditions
- Your dental membership fee is valid for one year from the membership start date.
- Initial memberships will start the day your membership fee is paid. Renewals will commence on the same day of the following year.
- Payment is due at the time of service for dental services not covered by this plan.
- Patients have the right to cancel their membership within 30 days, provided they have not used the plan. The membership fee is not refundable after the plan benefits have been used.
